It's been a hectic few weeks for AS Monaco and the confusion showed as the Ligue 1 club submitted their teamsheet for the weekend's game against Strasbourg. Despite the official sheet bearing the signature of newly-appointed head coach Thierry Henry, Monaco still made the clumsy mistake of listing Leonardo Jardim as their manager. Jardim was deposed on Oct. 11 with the team languishing down in the bottom recesses of the French top tier after a disastrous start to the season. The botched documentation seemed to serve as an omen for Henry's first game in charge as Monaco slumped to yet another poor defeat -- 2-1 at Strasbourg -- thus leaving them in 18th position and winless in their last 10 league outings.  Of course, Monaco's gaffe isn't the first teamsheet to go awry, with Chelsea appearing to use templates from the previous season when they accidentally replaced Andre Villas-Boas' name with that of ex-boss Carlo Ancelotti for a League Cup game in 2011. There was also Torino's misguided attempts to welcome Joe Hart aboard ahead of his debut for the Italian side by getting the on-loan goalkeeper's name completely wrong. Surely the most hurtful example of all must be Manchester United making a complete hash of Michael Carrick's name in the programme for the long-serving midfielder's own testimonial. Just to make a cringe-inducing situation 10 times more embarrassing, Graeme Carrick, Michael's older brother, had his name spelled perfectly correctly after being listed among the substitutes on his sibling's big day. Yeesh.
